Acute Care Services
Many short term health conditions can easily be assessed and treated through telehealth care by our pharmacists.
This includes conditions such as Bladder Infections, Cough, Cold, Flu, Yeast Infections, Nasal Congestion, Allergies, Cold Sores, Headaches and many more.
Refills and Care Transition Management
Our pharmacists can provide refills of non-controlled medications when you are unable to get in to see your normal provider or are transitioning between care settings or providers. Our team will assess if it is safe and appropriate to continue your existing dose or if additional monitoring or an adjustment is needed to ensure a seamless transition to your regular care team.
Preventative Health Services
Our pharmacists can help you get and stay well by working with you to provide the care and education necessary to prevent the onset of new diseases or complications of existing conditions.
Examples: HIV Prevention (PrEP), Weight Management, Smoking Cessation.
Chronic Condition Management
A wide variety of chronic medical conditions may also safely be managed through telehealth. Our pharmacists are experts in post diagnostic disease management in a wide variety of chronic conditions. We can assist either on a short-term basis when you are in between primary care providers or as a specialist on your care team and coordinated with your primary care provider.
